Upon receipt of the referral and initial risk assessment, we use the following process
We work with children and adults with learning disabilities and autism and their families to assess their needs and develop person-centred care plans. This involves understanding their strengths, weaknesses, goals, and aspirations
We work with service users to develop care plans tailored to their needs. These plans may include support with activities of daily living, communication, social skills, and employment
We constantly work with various professionals, including social workers, nurses, and occupational therapists, to provide coordinated care and support to people with learning disabilities and special needs. This involves sharing information, developing joint care plans, and working together to address the needs of the individual.
We work with families and carers to provide them with support and guidance. This may include helping them with the needs of their loved ones, providing practical support, and advocating on their behalf
Service users and their families are involved in all aspects of their care and support. This includes giving them a say in developing their care plans and the choice of services they receive
We support service users and their families to plan for transitions, such as moving to a new home or starting a new job. This involves helping them identify and prepare for the challenges they may face and ensuring they have the support they need to make a successful transition
We provide respite care for caregivers and their families. Depending on their needs, this may include family holidays, overnight care, weekend support, or day breaks
We provide a specialist ECS service for most local authorities and police forces throughout the UK. We receive referrals for young people who cannot stay in their family home for their safety or the safety of family members. When we receive a referral, we will assess their needs and source suitable support workers to support the young person for the duration required until a permanent solution is identified by the local authority
We provide supported living for care leavers aged 16-18. This includes accommodation, support, mentorship, and skills training for the duration of their stay. This usually includes support with attending further education in some form
We provide live-in care for adults and the elderly within their own homes. The support needs of our live-in care clients are usually profound and complex. We tailor our support to their individual needs
We provide supervised contact as required by families, the courts, solicitors, and local authorities
Our organisation takes training seriously. We have two accredited trainers on staff who provide training throughout the year, including individual support for complex cases. One trainer specialises in mandatory training, while the other specialises in Non-Abusive Psychological and Physical Intervention (NAPPI)
